REHABILITATION
& THERAPY SERVICES
Rehabilitation provides a vital bridge between hospital and home, offering care and support to help patients overcome the challenges of recovering from an injury, illness, or surgery. Our therapy services consist of physical therapy for mobility and strength restoration, occupational therapy to assist with daily activities, and speech therapy to rebuild communication and confidence. Each resident receives a tailored plan of care, delivered by a skilled interdisciplinary team, with therapy available seven days a week to maximize progress and clinical outcomes.
WOUND HEALING CARE
From the moment residents arrive, we carefully assess their skin health and nutritional needs to prevent and address wounds. Our team of specialized physicians, nurses, and therapists works collaboratively to treat a wide range of wounds, including surgical wounds, diabetic ulcers, pressure injuries, and chronic or non-healing wounds. With tailored, evidence-driven care, our goal is to promote faster healing, lower infection risk, and prioritize patient comfort. We strive for exceptional outcomes in wound healing.
PULMONARY CARE & SUPPORT
Pulmonary Care & Support services are designed to help residents maintain optimal lung health and breathe easier every day. Our skilled respiratory therapists work closely with physicians and the interdisciplinary care team to provide personalized treatment plans, including breathing exercises, airway management, oxygen therapy, and education on lung health. Whether recovering from an acute illness, managing a chronic condition, or seeking preventive care, residents benefit from compassionate, evidence-based support tailored to their needs.
LONG TERM CARE
Fairport provides high quality long- term care for those requiring 24/7 support and supervision. We focus on creating a safe, comfortable, and engaging environment where residents can maintain independence, dignity, and a high quality of life. Our skilled nursing staff, along with therapy and support services, work together to meet each resident’s unique needs, including medication management, chronic condition support, and personal care. Residents also enjoy three nutritious meals daily in a family-style dining setting, along with a variety of social, recreational, and spiritual activities. The goal is to ensure each resident feels welcomed, supported, and truly at home.
ASSISTED LIVING
Our Assisted Living community offers spacious, comfortable apartments designed to feel at home. Residences are grouped into intimate neighborhoods that provide the independence seniors desire, along with the safety and security they need. You won’t find long hallways or institutional nursing stations. Each neighborhood consists of 10–12 residents, creating a warm environment where seniors can socialize, build friendships, and enjoy meals together.
We offer 24/7 support for individuals who can live in an apartment-style setting but require assistance with meals, housekeeping, laundry, medications, bathing, and personal care. Each of our three floors features a community dining room with a fully equipped kitchen serving three meals daily, as well as a parlor and a shared living/activity space. Light housekeeping and laundry services are included.
Residents are welcome to bring their own furnishings to make their apartment feel truly personal, and we happily accommodate one pet cat.
Seniors maintain control over their daily routines, remaining as independent and involved as they choose. In addition, residents have access to the Fairport Rehab & Nursing Center’s continuum of care should a higher level of support be needed, based on availability and individual needs.
Fairport is also a licensed Assisted Living Program (ALP) community for those who qualify medically and financially. Our ALP includes all standard assisted living services, plus enhanced support such as personal emergency response systems, nursing services, a variety of home health services, and case management provided by a Registered Professional Nurse.
